Changes in Circumstances (This form will allow you to inform the Council of any changes in your circumstances) 1) Click 'Download Changes form' 2) Save this form to the hard drive on your PC 3)Print off a copy of this form 4)Fill out the details of any changes and sign the form 5)Post the completed form to: Benefits Section,Treasurers Dept. Guildford Borough Council, Millmead House, Millmead, Guildford, GU2 4BB You can claim the cost of your postage back from the Benefits section. Your Housing Benefit and Council Tax Benefit is based on the information you supplied. If your circumstances change you must tell us because it may effect your benefit. You should let us know in writing within 4 weeks of the change. If you do not and the change means you would get more benefit we may not be able to pay you the extra benefit. In the table below are examples of changes which must be reported to the Benefits Section:
Important Note to Landlords If as a landlord you receive direct payment of Housing Benefit you have a duty to report any changes in circumstance that might affect your tenant's entitlement to Housing Benefit. As a landlord you have the right to appeal about certain details of the claim. The time limit is one calender month from the date of the decision letter.
